Preferred "Any Doc" PPO
(Hospital Network)

the "Any Doc" PPO plan,
policyholders don’t have to change
doctors to realize the advantage of a low
$35 office visit copay. Offering the
flexibility to choose any physicians while
saving money with preferred rates at
prominent network hospitals, this plan is
ideal for people who want to visit the
doctors of their choice with no network
restrictions.

Preferred Managed Indemnity (No Network)
